For the second issue, the hearing only one contact when two sign in,
check very carefully.  Skype combines the two into one announcement.
Use Alt+Shift+F9 to repeat the last notification, or better yet, press
the Command key (usually the right square bracket), then the letter n,
and then start tapping numbers counting up from 1 to go back in time
through up to the last ten notifications, to see if some of them are
actually multiple events in one notification.  I suspect this is
what's happening.  You will hear the word "Skype," then one or more
announcements without another word "Skype," depending on how many
notifications Skype put into one alert.

The history issue is caused by a combination of issues and might have
to be fixed by Skype internally.  In short, there is no MSAA for that
list, and there is an error in how focus and selection are handled by
Skype in it.  I have not found a reliable way to script around that
one yet, but I have definitely filed a bug against it for Skype
itself.
