Sending and receiving files with Skype 4.1 is very accessible actually: "Send File" is still a choice in a contact's Context menu, and you then pick a file as before. Or, I believe you can still find a file in Explorer and then use the file's own Context menu to Send To Skype, then pick a Skype contact from there. As for receiving files, an incoming file transfer shows up as an entry in the sending contact's message window. You arrow to that entry in the message list, then Tab from there to reach the Save and Cancel buttons.
The only thing we lost from 3.8 to 4.1 relating to file transfers, far as I know, is the ability to see transfer speed and bytes transferred. This used to require the JAWS cursor I think, but now it's a tooltip and harder to get to than it was. I consider this mildly annoying but not major. I did report it though.
