Hmm, not sure what to say there. I just used the Add Contact screen while ago myself to add and name a telephone number. The only problem I had was having to use the Pass-Key-Through command, JAWSKey+3, before pressing Enter on the name I assigned the number. I'm not sure if that was necessary, but JAWS would not leave forms mode otherwise for some reason. I have not seen that screen crash.
