I believe that under Skype tools menu there is a screen reader checkbox you might want to look in to. If you can't make the adjustment using jaws then try another screen reader or narrator. This also sounds like a jaws tech support call since Jaws and Skype is something I believe they support though not sure about version 5.6. Let me know if the UAC problem keeps bugging you and I'll see what I can come up with later today.
