I found it better to mark the linux partition as "linux"
(or other non-dos types) so M$ leaves it alone, maybe
it won't touch ones marked as ntfs?

I recommend GAG (after hearing about it on slug I tried it on
my laptop and love it, my only wish is to change the "icons"
to better resolution images).

or if you've got linux somewhere already, do mkbootdisk
or the linux router floppy disk
or the linuxcare business card cdrom (which I love)
or any bootable linux cd should do

mkfs (and fdformat for floppies)
(you don't really mean "format" the disk (you use fdisk to
layout the disk), you mean "make a filesystem")

(you can have one M$ partition (ie. "C drive") assuming
it can handle more than 2Gb or 4Gb or whatever)
you can do:
partition 1: linux /boot (say 100 Mb or so)
partition 2: dos/lose95/98/whatever
partition 3: linux /
partition 4: linux swap (100-200 Mb)
(or do partitions 3 and 4 the other way around)

last time I reinstalled lose95 for my chick, she had a 4Gb
FAT16 partition on one disk (I know it's not supposed to be
possible, I created it in linux for her and it works ok except
things like defrag/scandisk/etc crash with "divide by zero" errors)
and the lose95 installer wouldn't work cos it couldn't run
scandisk on that partition. Luckily that partition is on
a physically different disk so I just unplugged it (which
is probably safer anyway since I wasn't sure if it would
have got wiped by the installer).
