The simple workaround is to turn off "online mode", and then generate
the PDF. It will do what you were expecting it to do. It would seem good
advice to always verify the layout of your document before printing (be
that to paper or PDF).

There was also a bug where if you had auto spell check on the little red
squiggles would show up in the PDF. That's fixed as of 1.1.1 (as least,
it's fixed as of 1.1.53 of the Ximian version of OpenOffice which is
what we use). The simple work around, though was to turn off auto spell
check before generating a PDF.
