There is always phpSurveyor which is a very powerful web based tool
which builds many different types of questionnaires including multiple
choice, "how do you rate" etc.  It is fairly easy to use and can produce
statistics.  It is (obviously) php and mySql based - easy to set up as
well.  See http://www.phpsurveyor.org/
I can see why it is obviously PHP based, but why would it "obviously" use MySQL - it could use, for example, Postgresql...
