drag them into the sidebar at the right place in icon view.

warning -- i've had pdfs became unstable and unsavable doing this. save each 
time you add a page, or frequently.

ah -- except that in Snow Leopard it adds them strangely, as a kind of single 
multiple page in one page. I think it was 'as expected' under Leopard. I think 
Automator has some stuff that might work.

I usually extract all pages via Acrobat as single pages, then open the first 
one in Preview, then drag all the others into the sidebar. This avoids Acrobat 
adding them all in the wrong order and other weirdnesses. But the whole thing 
is a bit clumsy.

I had this sussed until Preview changed in Snow leopard!
