In case anyone has similar problems to me with iTunes, I've combined my iTunes with EyeTv in Devonthink Pro. It's two different workflows and it's working a lot better.

1) iTunes:

Index (not import) all items in the Movies and TV folders, then sort either by smart groups or manually with folders. Periodically update the index (it's important to index the folders in the Finder to get this to work). So you have a folder that corresponds to the Movies folder, and another to the TV folder, and you update both periodically.

2) EyeTV

Initially also index the list of recordings (i.e. 'EyeTV Archive' folder) into a particular folder in DV (e.g. 'EyeTV inbox'). Then sort and tag as per the iTunes content and - if suits - use contextual menu to Move things into the database (but be careful not to do so with schedules or it won't record them when the time comes).

If you end up with loads of unwatched stuff this is actually better: you can see better what you have, sort it a bit instead of scrolling past all those documentaries;)

EyeTV only keeps track of the file until you quit it. If you do too much chopping and changing (e.g. deleting) within DV, it loses track a bit ad you have to quit it and relaunch.

Drawbacks: DV Pro can't - unless you manually add tags - keep track of what you have and haven't watched.
But so far it's a vast improvement on the iTunes interface...
