Have you tried repairing Macintosh HD with Disk Utilities? If the file system allows (ie HFS+), I'd run Diskwarrior on it via your external drive.

Failing that, I think I would try installing the OS having booted up from the external, onto the HD.

Good luck (is this time to ask whether it's regularly backed up? Possibly not... but if you have the capacity to take a snapshot of the current drive somewhere safe, I'd recommend that)
