I have seen a few apps that very explicitly say a folder will not be backed up (e.g. goodReader)... from this I infer that the default is to include all data. After all, plenty of applications don't use iCloud, and you would expect them to have data backed up.

It should back up any app data, so if an a cloud app like Dropbox has made any folders available offline by storing them locally, they may be included too, or not if the Dropbox app is clever and excludes that local data specifically from a backup
