You say nothing about your hardware.

The first thing I would check is RF quadrature.
If it is not OK, then nothing works as expected.

Do you use a digital phase shifter (74xx74 or similar)
or a discrete L-C-R network?

The digital shifter almost always works well. The discrete
one is harder to adjust properly.
