I use Takeoff Video <http://www.takeoffvideo.com/>. It allows the client to make comments at specific timecodes. I have been using it for about 3 years, now. Inexpensive, and reliable. Uses fairly good quality MP4 compression and will play in mobile as well as PC and MAC.
