The SpamAssassin custom rules mentioned on this list plus razor and DNS
tests (no RBLs) hit about 96%+ of my spam.  

Before, I used site-wide bayes, but it wasn't very effective due to the
diversity of my users.  Recently, I've set up bayes so that each user
has their own database.  People have commented that they haven't seen an
escaped spam in a while, but I'll have to wait a little longer for hard
numbers.  

I'm currently using a 100% open source solution: qmail-scanner + clam +
spamassassin on qmail + vpopmail + courier-IMAP.  What problems did you
have with qmail-scanner?
