This could be from 2 reasons I've seen / done.  

1) Did you use a patch for qmail-scanner? IE did it have sa-alt config flag?

or

2) In your /var/qmail/bin/qmail-scanner-queue.pl what is your
"spamc_options" ?

Mine is "my $spamc_options=' -f';"  Remove the -c which gives it a status
code.
