happened to me too once, couple of years ago, I can't remember why, but it was painful, discovering a queue of over 10,000 messages. i take your point. i was glad as it was in fact the BL that alerted me. just recently i got blacklisted by one host because an infected machine on an internal network sent 100's of virii, i only had File::Scan installed, which wasn't scanning zips. actually this issue is a little off topic for spamassassin, as i was blacklisted not only for the mail sent via relay, but also the ones the virus smtp engine sent, via NAT (nat host is smtp host)This reminds me of the day *I* had an open relay. :-(
