Martin I'm running SURBL with the score now set to 5.
The effect I've seen is that mail that used to score in my deliver but mark as spam range (between 5.0 and 9.99) has been much reduced. SURBL has effectively pushed these into my don't deliver spam range (10 +) I've not seen any false positives with SURBL which is why I've moved the score higher as I gained confidence with it. But, as usual, YMMV.
