In our config, I check against a user list and the access.db (sendmail)
because we're also doing some virtual domain stuff.  Some power users have
procmail set up to their own tastes because they live in both Windows and
*NIX environments but the majority of the email gets forwarded straight to
exchange.  Email scanning with SA is opt-in in our environment.

We don't check any of the outgoing mail.
