i've had the exact same problem. One or two emails per month go

I double checked my servers before posting to this list. I read your 
post but my servers are low on utilization. So excess cpu seems not be 
the reason for it. Also I have checked all of this specific suppliers 
mail and it is exactly only this type of mail that is ignored.
I can see the incoming at the qmail smtpd, see the transfer through 
qmail-remote to the Mailscanner and than the entry on Exchange. Weird...
hasn't happened for some time now, so all i'm saying is that i've seen
this before.
if you search the mailing list archives, some people have suggested
lots of alternatives on how to track and kill this problem.
... Search? What kind of keywords. I'm pretty new in this list so I have 
 no idea what kind of Subject or keyword I should use for this specific 
behavior.
