Does anybody have a concept why this is happening? Any thoughts is greatly appreciated.

We have Spectra 1.0.1 and we have been having problems with our page header images being deleted or "depublishing" randomly from our live pages. This seems to only happen on the index pages. The page header image is in a container by itself separate from other sections of the page. It has been quite a challenge trying to find out what may be causing this deletion since its behaviour is random but it deletes quite a bit of images and quite often leaving us with broken images on the live pages.
Does anybody have a concept why this is happening? Any thoughts is greatly appreciated.
