Randomly deleting page header images

We have Spectra 1.0.1 and we have been experiencing problems with images/graphics being deleted randomly from the page header container in our live pages. This has been happening for quite some time and it has been a challenge trying to find out what maybe causing this. Couple of things to note: 1) This seems to only be happening on index pages 2)the page header image is in its own container separate from other containers 3)it only deletes the page header images (other images in other containers like the ones in the body containes are fine). Again, this "voluntary" deletion is random but happens quite often leaving those pages with broken images.

Does anybody have a theory as to why this is happening? Any thoughts is greatly appreciated.
