Larry, Generally, 3rd + O/D is a gear that isn't used much, since the ratio is almost identical to 4th non-O/D. So, working up through the gears, you go 1, 2, 3, 4, 4+O/D. If you are pottering around town you are probably not likely to need 4+O/D, in which case it can be handy to drive around switching between 3 and 3+O/D, because that's less effort that changing up to 4th.
To remove the switch, you first pry off the plastic cap using a screwdriver in the slot handily provided. You can then disconnect the switch from the spade connectors. To remove the rest of the knob, use a spanner to turn the chrome cone-shaped bit at the bottom of the knob clockwise (i.e. down) - there's a couple of flats on it to enable you to do this. You only need to turn it enough for everything to become loose, you can the remove the nut that's down inside the knob, and finally the knob slides off.
