I believe it is supposed to be on the passenger side, but I will tell you
from experience that I like to put it on the driver side.
that is so that I can hit the switch with my left hand while shifting with
my right. not correct, but I think more functional

Can't help much with your other question but as far as where to fit the
long OD switch, in the UK it's on the same side as the indicators switch -
one advantage being that if you hit the wrong switch when trying to go in
or out of OD it's less of a problem if you indicate than knock the lights
on or off.
