Curious about this:

When I add a new CD, and before a rescan, I use "browse by folder".

It's been my experience that Browse By Folder is based on the scanned
library because it does not do a "read" of the file system when it's used,
but simply offers a different 'view' of the scanned library. Is this how it
works for you? Sounds like it does not ... otherwise you'd not be able to
see any new files added to the file system until after a scan adds them to
the library.
