in my case when Ive had this problem it was always caused by a subset of files having a single tag field that was different. The hard part is that it may not be a field you even use. mp3tag program is useful for opening all files, selecting show extended tags, then you can see any differences. Another approach is to simply strip out all the tags and retag. Mp3tag can help there too.
