My, very short, experience is that scan for new doesn't actually remove things. And doing a full scan doesn't always either. My method has been point the server at a different directory, do a new scan, validate 0 files, point back at normal, do a new scan.
This appears to do a full rebuild of the database properly for me, ymmv. I have only been using LMS for about two months. I'm running on a Linux server, and running the nightlies builds. Maybe file creation times are wrong, or in the future, so the scanner re-adds?
