I don't know the answer but some things to check ...
File name ... does the file extension match what LMS has been configured to look for? File permissions ... is the file accessible by your LMS? Do other flac files that you have work without issue in your LMS? Does the LMS scanner.log have any clues? Can you see and play the file if you browse in LMS from My Music/Music Folder ?
