i have run a utility on the server that assigns the following permissions
Can't you ssh to the box and the the permissions on those files? I have no clue what files or folders that script would touch. Just "ls -l /path/to/your/OPUS/file" and it's parent folders.
It's not clear to me whether that script you mention did change anything, whether you're still seeing issues or not.
