1. Do not use the recommended no_cache acl lines. Instead use these
refresh patterns:
refresh_pattern \? 0 0% 4320
refresh_pattern cgi-in 0 0% 4320

2. When the above is set up, change the CGI programs to return correct
expiry or TTL information or add specific patterns matching the programs
you want to cache.
