I found the problem.  It only happens if the URL that is being used is
IP address instead of the domain name.

By the way, what Free/Open Source software that you can recommend to me
for the administration of qmail + VMailMgr + Courier-IMAP + SquirrelMail
setup.  A web based app is very much appreciated.
