Michael - I updated the folder creation list for courier to exclude INBOX. (i.e. the default selection is now None, which has 'INBOX.' automatically prepended as the default prefix). This is consistent with what is present in the other folder options, also (delete/rename) where INBOX is not listed.
This eliminates the possibility of creating a nested INBOX folder, which I think was the heart of your issues - and I agree. That was a bug that should have been pointed out long ago. If you could pull down the latest CVS (or at least src/folders.php) and give it a whirl, I'd like to know what you think.
