Sorry I missed out the no.1 info.
1. Fyi, Im using plain userdb file, without SQL. I have configure my
courier-mta to deliver local mail using settings as follows:
Use Custom Delivery Instructions:
|| /usr/lib/courier/bin/dotforward
./Maildir

However, after change the settings it still doesn't work. But normaly mail
delivery still fine. Any idea?

2. Where can I enter the user login username and password for vacation? In
which file will do?
Do you mean every IMAP user has to define their own FTP account?

Plugin should work with .forward or mysql

courier has to be setup to use .forward files. man dot-forward
You haven't said anything about courier with mysql support.

Plugin uses username and password that were entered when you logged into
squirrelmail.

Take a look at themes/css directory. Stylesheets are stored there.
