Run conf/conf.pl. Chose 4. General Options, then 6. Usernames in Lowercase. Turn it on. Or just edit config.php and change:
$force_username_lowercase = false; to be true. This will cause Squirrelmail to change all user names to lowercase no matter if they type USER, User, or user. user.pref will be the only pref file created/used.
