Now I have this same config running on a Suse Linux Computer with no problem..
I did have this issue before and I made a simple change and totally forgot what it was.

Check if file /usr/local/www/squirrelmail/functions/abook_database.php exists and if the user the web server runs has read perms(and maybe execute perms) on this file.
