I was able to make the problem go away by adding unset($theme) before the line that was failing in config.php.

I don't know why this was an issue but my guess is that somehow $theme was somehow defined before and that indexing using a string didn't make sense according to that definition.

disable register_globals for squirrelmail install or put
'var_dump($theme);' before $theme[0] lines in config.
