When I forwarded the message to someone in the ISP's tech support to complain, his email client (Outlook) correctly picked up the attachment in the forwarded message.
Any ideas what might be causing this?
It's a uuencoded message. Outlook automatically decodes them before dealing with them.

Ken is right about being able to download and uudecode it but I think there is an actual attachment issue with Squirrelmail.
I haven't dug into the details of SM but I've got several hand crafted email messages (for testing mail servers and MUAs) that I belive are 100% RFC compliant which fail on SM but work find on Outlook, Mozilla mail, iMail (Apple), and KMail (KDE).
Here is one example (with a uuencoded attachment):

Notice the string specified in the boundary="..." bit in the headers. That string should not occur anywhere in the body except as part of the mime part delimitters.
