If you put the patch in the main SquirrelMail directory, this should work:

squirrelmail-1.4.4]# patch -p1 < sqm-144-xss.patch

If you for some reason cut and pasted the patch instead of downloading it (wget or whatever), you might add the -l option.

Perhaps you used the wrong -p or --strip option?
