I've seen a similar symptom before, but unrelated to SM. I've noticed that sometimes (always?) when I upgrade php I get this error until I fully stop and restart apache. I'm guessing you probably did this when you said you restarted apache after you made config changes, but I'm pretty sure graceful's aren't enough so I just thought I'd confirm.
