The first thing to look at is at the mail headers, so that you actually know where the mail comes from, or where it is supposed to go.
issue 2
in the case where the email just "disappears" randomly, is there a way to actually look at the /var/log/maillog for those instances? i can't imagine tailing the log file and watching every email that gets sent out to confirm its delivery or disappearance..
