No, I think you might be missing the point of the patch. It's not designed to eliminate stylesheets (they're a Good Thing), but rather it's designed to allow templates to be more "pluggable". With this patch, I can drop my template into any two directories (one for html, one for images), set the environ variables SQWEBMAIL_TEMPLATEDIR & SQWEBMAIL_IMAGEURL appropriately, and the template automatically works. Otherwise I'll have the additional step of modifying any background image locations in the stylesheet on a per-template basis for each template install. Does that make sense?
