It's a bad idea to append the number to the namespace since it reserved
for different revisions of XEP and not for your purpose. Again, I think
that this should be solved by some privacy lists extension since your
decision again restricted.

Client should cache data. You will flood anyway the only difference is
who will flood: client or server.

Again, you will receive serial numbers for the pubsub journal and you
can retrieve all the data to make your cache up to date without
overhead. That's my decision, maybe some better exists.

There are GIT repo for XEPs and other stuff:
git://gitorious.org/xmpp/xmpp.git but I don't know this process in
detail too.
