When I had these symptoms on my 4200DT it turned out to be a bad PCI riser card. At first I suspected the Voodoo3 PCI graphics card, since the problem went away when I removed it. But it worked OK when I replaced the 3-slot riser, as did everything else.


You could try taking out the riser to see if the 'Max will boot - mine worked without one for a while. I got a replacement on eBay which I *think* came from a Power Mac 4400 - it had a grey metal plate I had to remove so it would fit. But I've since seen 4400 risers that look somewhat different from the StarMax ones, so care is needed. Five-slot risers I know nothing about.
