The FC HBA is a QLogic QLA2310F and the latest available driver is installed.

Tests have also been conducted using USB and SCSI drives, but with capacities 
less than 500GB, so the cylinder overflow problem is not seen. What is 
interesting is that because all these drive combinations have been configured 
as "hot-swappable" devices, this seems to set the head/sector count to 255/63 
(but this hunch may not be correct?).

Tests with larger capabity drives (>500GB) are being tried to see if the 
problem can be narrowed down to a single type of controller, or whether it is a 
larger scoped issue.

I hadn't filed an OpenSolaris bug because we're not using OpenSolaris, and so 
have no reason to believe that the bug does (or does not) exist in that 
platform. Are you saying to file an Open Solaris bug report regardless ?
