Based on the trace its definitely the Target terminating the login request. This is seen by a TCP level disconnect (FIN) at frame 12. I would expect the target doesn't like something in the (Operational) Login Request (Frame 10). Does the target debug log have some sort of error that points to the field it doesn't like or maybe a core file? Nothing is jumping out at me. (Off Topic...Those initiator requested transfer sizes (4K and 8K) are ridiculously small, but legal. That should hurt the initiators performance some.)
Not knowing the target code I would guess your making it to around line 526 in iscsi_login.c and your encountering a problem somewhere after there (Just making wild guesses). It looks like most failure paths would issue send_login_reject() and since you don't see that your in some corner case or the target crashed?
