You should have a look at the output from "fmadm faulty"
and "fmdump" to see whether there's anything been logged.

Apart from that, is there anything in /var/adm/messages?

Finally, as far as I'm aware we haven't got the binfix
ready for the issue you and Robert have logged, so the
-15 and -16 revs of the patch aren't useful to you for 
enabling MPxIO with pci-e hbas and more than 4Gb of ram.
