Unfortunately, the problem is that the LP1050EX-E adapter you are using is considered a "mid-range level" adapter (less expensive model) and the Solaris driver is designed to only operate with "Enterprise level" adapters (more expensive models). The driver is technically capable of supporting it, but the firmware on the adapter will not allow it to initialize with the driver. The 0xf0a5 device id listed in the driver_aliases file was put there to support a special enterprise level 2G Blade adapter we also sell. It is only coincidence that your mid-range adapter also had the same device id, which allowed the driver to attempt adapter initialization.
