I just spoke with a gentleman at IDS -- and, indeed,
it was they who were contracted to put in the
"insert."  He claimed it was not an insert because it
was not literally (or at least wasn't supposed to be)
literally inside of the newspaper.  Mine was inside
the bag along with the Voice, so the difference, as he
accepted, is semantic.  

He made a point of saying repeatedly that the Voice
had nothing to do with it.  My guess is he probably
got a phone call from the publisher asking him why he
got him into hot water.
