Hello,
for possible reasons that could cause collisions, see

It's always a good idea to observe the situation in question via sumo-gui
and also some seconds that lead up to it.  To see the collision as it
happens, set '--collision.action warn'
Collisions on junctions are computed by geometrical overlap and are subject
to errors in the network geometry (i.e. miscomputed waiting positions
within a junction).
Furthermore collisions may also be caused by bugs in the model.
