This is a KDE-only problem, yes? The scroll wheel works in Gnome, and it reports ButtonPress and ButtonRelease events on buttons 4 and 5 to 'xev'? If this is true then the problem has to be in a KDE (QT) library. Maybe there's something about the events that QT doesn't like.
