We have XKB turned off and occasionally experience this issue with
srss4.1 on debian. I can't really find a specific trigger issue, it
just seems to happen every once in a while.

4.1 patch, due out in a few weeks. In the meantime you might try turning off
the XKEYBOARD extension ('utxconfig -k off') for people who run into this
problem. They'll need to log out and then log back in to put that setting into
effect.
If XKEYBOARD is already turned off then this would be a new issue.
